Service Description
We accept both male and female residents, including Medicaid and private pay. Private pay rates are available upon request. Our home provides a safe, comfortable, and supportive environment where each individual is treated with dignity, respect, and compassion.

Level of Care & Acceptance Criteria
To ensure safe and appropriate care, our home supports:

Level of Care: Two-person assist
-
Transfers: We Accept Residents Who Require one or Two-Person Assist, Including Use of Gait Belts, Sit-to-Stand Devices, Transfer Boards, and Hoyer Lifts, When Care Needs Can Be Safely Met.

Mobility: We accept residents who are independent, use walkers, or use wheelchairs, as long as they can be safely assisted by caregivers

Behavioral Criteria:

We accept wandering residents

We are not able to accommodate residents with aggressive behaviors at this time

We do not accept residents with suicidal behaviors
Age & Capacity

We primarily accept residents age 60 and above

Residents under 60 may be considered depending on individual needs and level of care

Our home is licensed for up to 3 residents

The shared room may be offered as a private room depending on the resident and care needs

Staffing & Clinical Support

Unless two caregiver are needed, one caregiver is on-site at all times. two will be on site all times if client needs 2 person assist

Care is structured around one-person assist for safety and consistency
While we do not have full-time nursing staff on-site, we can coordinate licensed nursing services as needed, depending on the resident’s care plan and DSHS approval.

We can assist with arranging transportation for medical appointments and other essential outings upon request, depending on resident needs, staffing availability, and advance scheduling.

Same-day or next-day admissions available, depending on fit and medication/nursing delegation requirements

We typically respond within 10–15 minutes to calls, texts, or emails

We quickly review referrals and provide clear acceptance decisions

We provide required documentation promptly upon request

We maintain clear and honest communication about fit to ensure appropriate placements
We work closely with case managers, families, and healthcare providers to ensure smooth and coordinated transitions.
We are also listed on Room & Care, where case managers can view our home. Families and others can access full details through our website.
